Registration Procedure

If you are a NEW player:

Provide ALL FOUR components below to your team representative. Your team rep submits this packet to the state registrar who will issue a player pass - processing can take 3-5 days.
You cannot play without a pass. PERIOD.

1. TWO copies of a completed and signed registration form (download below).
2. One color near wallet-sized head photo with player name written on the back.
3. One photocopy of a photo ID (i.e., school ID, drivers license, state issued ID).
4. $30.00 registration fee (Your team may charge more for dues to cover the cost of running the team).

If you are a RETURNING player, you need TWO signed copies of the registration form (download below) submitted to your team representative.

Note: Registration closes the fifth game into the season.
