RIWSL Compact Registration Form (PDF)

This form is used by the team representative to collect signatures from RETURNING PLAYERS ONLY. The compact registration form is submitted in lieu of individual registrations from RETURNING PLAYERS ONLY. This form CANNOT be signed by NEW players to the league, or by any returning players whose personal information (address or phone number) has changed.

Scorecard/Lineup Sheet (Excel file)

Use the above link to download scorecard/lineup forms. This form must be completed prior to game time and submitted to the officials and opposing team before the start of each game. This is an official document. FILL OUT COMPLETELY AND ACCURATELY.

Insurance Forms for Fields (Excel file)

This form is completed once per season and is required by all towns, RISA and RIWSL.
